Completing the IP Justification Form

The IP justification form is a requirement from ARIN, which is regularly audited. Each form submitted will be manually reviewed for accuracy. You must provide valid data to receive additional IP allocations.
 
The IPv4 request form is split up into four sections.
 
Requested Subnet Size: From this drop down, select the IP block size that you're requesting, this is in addition to any IP blocks you currently have assigned to you. 
 
/29 - 8 IPs, 5 Usable
/28 - 16 IPs, 13 Usable
/27 - 32 IPs, 29 Usable
/26 - 64 IPs, 61 Usable
/25 - 128 IPs, 125 Usable
/24 - 256 IPs, 253 Usable
 
Existing Assignments: You must provide details about IP blocks that you have been previously assigned. For each IP you must list the customer that the IP is assigned/re-assigned to, what the IP is being used for, and a domain/hostname if applicable. The customer must be either a valid full name or company. The usage should be something that absolutely requires it's own IP and a service/usage that cannot be shared with any other IP due to technological limitations. For any IPs that are not usable, please either list not usable or N/A.

IPs utilized/projected for: Categorize the IP usage based for the additional IPs you're requesting. If you requested a /28 (16 IPs), you must list how the 16 IPs will be utilized along with any future utilization. Note that 3 IPs are already categorized/reserved in the subnetting portion of the form as those 3 IPs are not usable and reserved for networking purposes (gateway, broadcast, etc). The numbers under the Imm/Immediate column must add up to the IPs you requested.
 
Comments/Remarks Fields: If you categorized IPs under "IP based Virtual Hosting" or "Other", then you must further describe in a brief summary about that specific IP usage. Additional remarks are not required, but is helpful for us to understand your usage and requirement for additional IP space.
